Enrollment Forms 2026-2027

Be sure to go to the Enrollment & Registration Information 2026-2027 page to see Important Dates & information.

Please print the forms or come to the school beginning March 20 , 2026 to pick up the forms packet. We will begin accepting enrollment papers in the Main Office beginning April 13, 2026 from 8:30 a.m. to 10:30 a.m. and 2:30-4:00 p.m. PLEASE do not upload the forms online.

Incoming 6th graders and students new to Holmes need to come into the Main Office to submit the required documents.
Returning 7th & 8th graders will turn the forms during Registration Day in August.

If you upload them online, please bring a copy of the ORIGINAL forms on your registration day in August.

Field Trip Forms Health History Form (REQUIRED)
Parent/Guardian Publicity Authorization and Release (REQUIRED)
Holmes Locker Contract (REQUIRED)
Responsible Use Policy (RUP) for District Computer Systems (REQUIRED)
Parent and Student Acknowledgement Form – Loaned Computing Device (REQUIRED)
Opt-In/Out for Loaned Device
Financial Responsibility for Damaged School Property Letter (REQUIRED)
Consent to Use Digital Instructional Tools
Culmination Activity and Certificate of Completion Policy (REQUIRED)

Please bring proof of an active LAUSD Parent Portal account (printout, login, screen shot of completion, etc.)
Parent Portal: Requires an active email account, Student ID and parent PIN to register. Contact your child’s current school for your parent PIN or call at (818) 678-4110 after July 1, 2026.
