- LAUSD Student Enrollment Form (ONLY REQUIRED for Students New to Holmes and Incoming 6th graders)
- Emergency Information Form (REQUIRED)
- Student Housing Questionnaire (REQUIRED)
- Holmes Locker Contract (REQUIRED)
- Parent/Guardian Publicity Authorization and Release (REQUIRED)
- Responsible Use Policy (RUP) for District Computer Systems (REQUIRED)
- Parent and Student Acknowledgement Form – Loaned Computing Device (REQUIRED)
- Financial Responsibility for Damaged School Property Letter (REQUIRED)
- Additional Forms, if applicable (Immigrant, Migrant, American Indian, Refugee, see below)
Please submit proof of an active LAUSD Parent Portal account (printout, email, screen shot of completion, etc.)
